Iran's recently-installed Supreme Leader Mojtaba Khamenei is unconscious and receiving treatment for a "severe" medical condition in the holy city of Qom, according to a diplomatic memo based on US and Israeli intelligence seen by The Times of London and reported on April 7.

Although unconfirmed, the reports add to mounting evidence of an effective military takeover of the country in the past month, sidelining the traditional structures of power, with the supreme leader unable to participate in any decision-making.

"Mojtaba Khamenei is being treated in Qom in a severe condition, unable to be involved in any decision-making by the regime," the memo reads, according to The Times. The document was shared with Gulf allies and reveals the Supreme Leader's location for the first time since the war began.

Mojtaba Khamenei, 56, was installed as Supreme Leader after his father Ali Khamenei was killed in the opening US-Israeli strikes on February 28. He has not made a single public appearance since taking office. Iran has said he sustained only a minor leg injury in the strike that killed his father, mother and other family members.

So far, the only moving images of him were released on April 5; however, the video purporting to show the cleric walking into an underground bunker was widely dismissed as AI-generated.

 

Two statements have been issued in his name during the war, both read by presenters on state television. A video released on April 6 appeared to show Mojtaba entering a war room and studying a map of an Israeli nuclear facility, but was later identified as AI-generated, The Times of India reported.

A leaked audio recording obtained by The Telegraph from the February 28 strikes revealed that Mojtaba escaped death by minutes. Mazaher Hosseini, head of protocol for the Supreme Leader's office, was heard saying that Mojtaba had left his father's side moments before the missiles struck the palace.

Russia's ambassador to Iran, Alexey Dedov, dismissed claims on April 1 that Mojtaba had been secretly moved to Russia for treatment.

Intelligence agencies have also observed preparations for a large mausoleum in Qom with space for multiple graves, the memo said, suggesting the burial site will house Ali Khamenei and family members killed in the strikes.

Iran's foreign ministry said on April 1 that Mojtaba was "completely healthy" and that not appearing in public was "not unusual under wartime conditions." The question of who is running the country's war effort in his reported absence remains unanswered. Parliament Speaker Mohammad Bagher Ghalibaf, President Masoud Pezeshkian and the IRGC command have all issued public statements and operational orders throughout the conflict.

The claims in the memo should be treated with caution. US and Israeli intelligence assessments have been used throughout the war to shape narratives about Iran's leadership and military capacity.

Tehran has consistently denied reports about Mojtaba's condition.
